Sheppard Pratt is seeking a School-Based Case Manager to provide culturally competent, comprehensive case management for school-based mental health in collaboration with the managing department. The role supports youth and families, promotes self-sufficiency, and links to resources while maintaining client confidentiality.
Schedule: Monday–Friday, 9am to 5pm. In-person work; requires a BA/BS in social work, counseling, psychology, or a related field and 1 year of prior experience.

What to expect.
Provide culturally and linguistically appropriate and comprehensive case management services for the school-based mental health in collaboration with managing department. The services are to promote self-sufficiency skills, family development, and linkages to mental health resources which are targeted to youth and their families. Collaborate with the clinical treatment team, school staff, and community partners in providing comprehensive care while maintaining strict confidentiality of client information. Provide early intervention and prevention services to students who are identified by therapists, counselors, and other school-based staff.

The pay range for this position is $27.23 hourly minimum to $29.06 hourly maximum and is determined on a number of factors, including but not limited to, years and level of related experience.
