Job summary

Palmer Continuum of Care, Inc. in Oklahoma is seeking a Behavioral Case Manager II to provide individualized and group education, develop care plans, and coordinate services for adults and adolescents in treatment.

You will follow ODMHSAS standards, maintain strict confidentiality, and document case management records. You will link consumers to community resources, advocate for their needs, and contribute to ongoing staff development.

Qualifications

• Current Behavioral Case Manager II certification by ODMHSAS.
• Experience in providing individual and group education.
• Previous experience in addiction recovery services preferred.
• Previous experience with adolescents preferred.
• ASAM PPC-2R trained by ODMHSAS-certified trainer within past 5 years.
• Ability to develop and maintain collaborative relationships with referral sources.
• Excellent verbal and written communication.

Job description

Summary

Empower consumers to access and use needed services to meet self-determined goals. Plan referrals, linkage, monitoring and support, and advocacy assistance provided in partnership with consumers to support the consumer and their biopsychosocial needs while in treatment. Conduct educational groups regarding substance use and recovery to adults and adolescents.

Responsibilities

• Adhere to all Palmer goals, strategies, and values.
• Maintain strict confidentiality in accordance with Palmer Polices and state and federal laws.
• Role model appropriate behavior, professional boundaries, respectful communication, and personal appearance in accordance with Employee Dress Code.
• Follow standards set by ODMHSAS for Case Management Certification.
• Develop case management care plan, including discharge.
• Complete a strengths-based assessment and generate needs lists.
• Maintain case management documentation and provide complete and accurate records within time requirements of Palmer’s Policy and Procedure manual.
• Provide linkage, referral, follow-up, monitoring and advocacy between consumer and community resources.
• Plan, implement and facilitate educational and skills groups both in the office setting and at area schools. Keep records of participation for billing and outcomes measurement.
• Serve as liaison to schools and create partnerships to expand school presence.
• Serve as a member of treatment team and provide Case Management progress.
• Supervise consumer’s behavior, adherence to rules, as well as policies and procedures.
• Conduct educational groups in accordance with adolescent program school agreements.
• Maintain facility security and assist in basic crisis intervention.
• Participate in ongoing in-service education programs and staff development programs.
• Maintain knowledge of agency policies, procedures and training.
• Maintain core competencies in relation to working with substance use disorders through continuing education and implement skills learned.
• Perform other duties as assigned by supervisor and/or Director.

Requirements

• Current Behavioral Case Manager II certification by ODMHSAS
• Experience in providing individual and group education.
• Previous experience working in addiction recovery services, preferred.
• Previous experience working with adolescents, preferred.
• ASAM PPC-2R – trained by an ODMHSAS-certified trainer within the past 5 years
• Ability to develop and maintain collaborative relationships with a variety of referral sources.
• Excellent verbal communication, written communication
• Advanced interpersonal skills
• Strong Computer skills include Microsoft Word, Excel, and Outlook
• Current Oklahoma Driver’s License and acceptable driving record according to Motor Vehicles Records

Physical Requirements

• Ability to lift 30 pounds, sit for up to 8 hours/day, view computer screen for up to 8 hours/day.
• Job requires twisting, lifting, standing, sitting, walking, stretching, kneeling, bending, and squatting.
