Oklahoma Treatment Services, LLC is seeking a Case Manager II in Oklahoma City to provide direct and indirect casework, rehabilitation, and skill development for patients and families in our opioid treatment program.

The role emphasizes confidentiality, coordination with medical staff, discharge planning within 14 days, and active participation in treatment planning, education, and outreach to the public and professional community.

Qualifications

  • Bachelor's or Master's in a behavioral health field (or Education) and ODMHSAS CMII competency exams.
  • CPRP certification or equivalent Psychiatric Rehabilitation credential or progress toward licensure with ODMHSAS training.
  • CMII Certification issued July 1, 2013 or after.

Responsibilities

  • Provide Individual and Group Rehabilitation and Skill Development Services per ODMHSAS.
  • Submit forms to Medical Director for take-out meds, dose changes, phase changes, and MSW schedule.
  • Protect patient confidentiality; complete releases for external referrals.
  • Complete patient discharge within 14 days after discharge; file with Program Director.
  • Contribute to assessment and treatment planning through daily reports and reviews.
  • Educate patients about chemical dependency and recovery process.
  • Educate public and professional community about opioid treatment programs.
  • Participate in professional development and required in-service programs.
  • Secure urine samples for monthly urinalysis; document results.
  • Engage in Diversion Control via bottle recalls.
  • Manage caseload and identify community resources for patients.
  • Advocate for patients with providers and agencies.
  • Provide information to guardians/family for decision making.
  • Solve problems and mediate with providers; other duties as assigned.

Job Summary

The Case Manager II is responsible for providing direct and indirect casework services to patients and their families and rehabilitation and skill development services

Essential Functions
  • Provides Individual and Group Rehabilitation and Skill Development Services as described in the ODMHSAS Services Manual.
  • Provide necessary forms to the Medical Director, properly and accurately completed, when the patient has special requests for take-out medications, dose changes, phase changes, MSW schedule, and medication changes.
  • Ensures that patient confidentiality is protected by properly completing release of information to ALL outside referrals and contacts (i.e. Medical coordination, information from other facilities).
  • Completes discharge of a patient from program, and is to be completed within fourteen(14) days from the date of discharge. The discharge form is to be completed in full and the file to be submitted to the Program Director.
  • Contributes to the ongoing assessment and treatment planning process through participation in daily reports, case reviews, and treatment teams.
  • Provides patient education relating to the illness of chemical dependency and the process of recovery.
  • Provides education and public information services to the general public and the professional community regarding opioid treatment programs.
  • Participates in a program of professional development including both external and facility required number of internal in-service programs and events.
  • Work with the medical staff in securing urine samples for monthly urinalysis. Document results in the patient’s chart and in Methasoft program.
  • Active involvement in Diversion Control through execution of random bottle recalls.
  • Manages and prioritizes a caseload and provides case management services to patients and their families.
  • Assists the team in identifying community and agency resources to meet patients’ needs.
  • Ensures that necessary services are provided to individuals.
  • Acts as the patient’s advocate and intermediary and ensures that providers meet the needs of the patients.
  • Provides information to individuals/guardians/family members/advocates for decision-making related to service options to patients with appropriate releases.
  • Solves problems and mediates with providers, agencies, and other team members.
  • Other duties as assigned by Program Administrator.
Essential Qualifications
  • Possess a bachelor’s or master’s degree in a behavioral health related field; or a bachelor’s or master’s degree in Education; and completed ODMHSAS required training and pass CMII competency exams.
  • Possess a bachelor’s or master’s degree in any field and either a current certification as a Certified Psychiatric Rehabilitation Practitioner (CPRP), or a Children’s Certificate in Psychiatric Rehabilitation from the United States Psychiatric Rehabilitation Association or a Bachelor's or Master's degree in any field and proof of active progression and complete ODMHSAS required training and pass CMII competency exams.
  • Possess a bachelor’s or master’s degree in any field and proof of active progression toward obtaining a clinical licensure Master's or Doctoral degree; and complete ODMHSAS required training and pass CMII competency exams.
  • Must possess a CMII Certification, and the CMII Certification must be issued July 1, 2013 or after.
Physical
  • Frequent sitting at desk or computer while typing, completing paperwork, or meeting with patients.
  • Occasional lifting/carrying, paperwork, files, office supplies, up to 20 lbs.
  • Occasional push/pull exerting up to 5-10 lbs. force, opening doors, file drawers, pushing files in drawers
  • Occasional kneeling/crouching and/or ability to reach lower file drawers, file stretching to store or retrieve materials.
Mental

Position requires constant mental alertness, attention to detail, and high degree of accuracy required in completing all assignments. Must be able to follow oral and written instructions and follow-through on all assignments.
