Job Description:

The Scheduler supports Administrators (or Designated Branch Directors) in scheduling patient visits. By managing key workflows throughout the patient journey, the Scheduler coordinates direct care team member schedules to ensure timely and adequate patient care visits. The Scheduler acts as a liaison between the Clinical Team Manager (CTM), direct care team members, the patient’s physician or medical provider, and other facility staff involved in patient care.

Minimum Requirements:
  • High school diploma or GED required; Associate's degree preferred.
  • At least 1 year of practical experience.
  • Experience in patient scheduling within a home care agency is preferred.
  • Knowledge of national standards of care, state licensure, and federal conditions of participation.
  • Strong communication skills, including conveying information clearly and listening actively.
  • Ability to plan, set timeframes, and allocate resources effectively.
  • Decision-making skills, including handling incomplete information and escalation when necessary.
  • Proficiency in enterprise applications (e.g., Homecare Homebase, Workday) and intermediate Microsoft Office skills.
  • Conflict management and negotiation skills.
  • Patient advocacy skills, understanding patient needs, and managing expectations.
  • Ability to respond to unexpected situations proactively.
  • Understanding of the financial impact of scheduling decisions and staff productivity metrics.
  • Bilingual in Spanish and English is preferred.

You will report to the Administrator or Branch Director.
