Empath Home Health, a member of Empath Health, is seeking a Home Health Scheduler. The Home Health Scheduler is responsible for coordinating and maintaining clinical staff schedules using Empath scheduling software.

Location: Fully onsite at our Naples, FL office

Schedule: Monday-Friday 8:30am-5pm

The Home Health Scheduler

  • Maintains clinical staff schedules and notifies staff if one is not present.
  • Schedules client appointments/visits according to care plans/evaluations and staff availability.
  • Enters scheduling data into Homecare Homebase HCHB and creates schedules.
  • Demonstrates professional conduct and ethics according to Empath Health policy and procedure.
  • Maintains confidentiality of patient information and all Empath Health operations.
  • Processes all incoming patient referrals from physicians and acute/non-acute facilities.
  • Schedules clinical staff to clients determined by location and availability.
  • Assists staff with coverage for PTO.
  • Contacts care providers and clients regarding day-to-day changes.
  • Therapy visits are maintained with attention to 30-day re-evaluation and 13th/19th re-evaluation to include multiple therapies.
  • Performs general office duties including but not limited to word processing, photocopies, files, shreds, provides reception and telephone services.
  • Verifies Medicare benefit eligibility through on-line DDE checks; and if necessary, confirms beneficiary’s status with other agencies by phone.
  • Determines and communicates medical complexity/acuity of all referred patients and their orders to the Clinical Coordinator.
  • Coordinates patient care with various members of the healthcare team, including physicians, physician’s office staff, hospital discharge planners/case managers, and Territory Managers.
  • Serves as a resource for physicians, hospital personnel, patients, and family or caregivers in order to facilitate communication, answer questions and resolve problems with assistance of Clinical Coordinator.
  • Operates Homecare Homebase (HCHB), DDE, ECIN on-line to internet as needed, Fax, copy, etc.
  • Assists with scanning referrals and admission paperwork into the Homecare Homebase (HCHB) system as needed.
  • Performs other job duties and responsibilities as assigned by Empath leaders.

Requirements

  • At least one (1) year of experience in Home Health scheduling is strongly preferred.
  • High School Diploma or GED is required.
  • Currently licensed as a Licensed Practical Nurse (LPN) in Florida is preferred.
  • Demonstrates strengths in collaboration and partnership to attain strategic goals, business plan development and execution, organization, and follow-through.
  • Possess organizational, problem-solving, time management, and decision-making skills.
  • Advanced interpersonal skills including verbal and written communication skills.
  • Able to work autonomously with minimum supervision.
