Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable full-time Scheduler to support our Home Health & Hospice team. The Scheduler coordinates daily visits for nurses, aides, therapists and other clinical staff, ensuring timely, quality patient care across geographic areas.

The ideal candidate has healthcare scheduling experience, exceptional time-management and communication skills, and comfort with EMR systems.

Qualifications

• Healthcare scheduling experience preferred.
• Excellent organizational, communication, and time-management skills.
• Strong attention to detail and ability to manage multiple priorities.

Responsabilités

• Coordinate daily schedules for nurses, aides, therapists, and other clinical staff based on patient needs, staff availability, and geographic location.
• Schedule admissions, routine visits, and follow-up care.
• Communicate schedule changes with patients, families, clinicians, and leadership.
• Monitor schedules throughout the day and adjust as patient needs and staffing change.
• Assist with weekend, holiday, and on-call coverage.
• Maintain accurate scheduling information in the electronic medical record (EMR).
• Track missed, cancelled, and rescheduled visits.
• Assist with entering referrals and coordinating onboarding of new patients.
• Provide professional and compassionate customer service to patients, families, and staff.

Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable, compassionate full-time Scheduler, preferably with home health scheduling experience to join our Home Health & Hospice team. The Scheduler plays an essential role in coordinating patient visits and supporting our clinical staff to ensure patients receive timely, consistent care.

Responsibilities:
