We are looking for a dedicated, compassionate, and sharp operational talent to join our team in-office as a Community Care Coordinator (Scheduler). In this key position, you will sit at the very heart of our daily operations, ensuring that our client care runs smoothly, our caregivers feel supported, and our families experience seamless communication.
As the operational bridge between our field caregivers, registered nurses, clients, and internal leadership, you will do much more than simply assign shifts on a calendar. You will cultivate genuine relationships, anticipate scheduling needs, troubleshoot real-time adjustments with calm efficiency, and serve as a reliable voice of empathy for families navigating home care.
