BETH ISRAEL LAHEY HEALTH at Home in Reading, MA is hiring an LPN to join a Hospice team. The role provides direct nursing care under the Hospice RN Case Manager, delivering palliative care and symptom management per the plan of care.

The LPN will perform visits, observe patient status, educate families, and participate in IDT meetings, while upholding HIPAA and patient confidentiality. On-site daytime hours in a supportive, inclusive team environment.

Qualifications

• Active Massachusetts LPN license.
• NLNAC-accredited LPN program.
• Active BLS certification required.
• 2+ years as a LPN in acute care, rehab or home care.
• Hospice experience preferred.

Responsibilities

• Perform nursing visits to patients as assigned by the Hospice RN Case Manager/Hospice Clinical Manager.
• Provide hospice nursing care to patients and families per palliative care principles.
• Observe, record, and report significant changes to MD and Hospice RN Case Manager.
• Engage in health teaching for patient and family.
• Communicate with patients, caregivers and IDT members clearly.
• Attend regular IDT meetings.
• Maintain confidentiality per HIPAA guidelines.
