Beth Israel Lahey Health at Home is seeking a Hospice Licensed Practical Nurse to provide skilled nursing visits and direct patient care in a home setting. Under the supervision of the RN Case Manager, you will assess, plan, and evaluate nursing needs while delivering compassionate symptom management and comfort measures.

Applicants must hold MA LPN licensure, AHA BLS, and have two years of LPN experience in acute care, rehab or home care; hospice experience is preferred.

Qualifications

• Graduate from a Licensed Practical Nursing program accredited by the National League of Nursing (NLNAC).
• Current licensure to practice as an LPN in Massachusetts.
• Maintains AHA BLS certification.
• Two years of experience as an LPN in acute care, rehab or home care.
• One year of hospice experience preferred.

Responsibilities

• Performs nursing visits to patients as delegated by the Hospice RN Case Manager/Clinical Manager.
• Provides hospice nursing care to patients and families utilizing palliative care and symptom management.
• Observes, records, and reports significant changes in patient health to MD and RN Case Manager.
• Communicates with physician to report changes and obtain orders.
• Participates in weekend and holiday rotation as required.
• Document services using problem-oriented records; participate in hospice case conferences.
