Beth Israel Lahey Health at Home seeks an experienced Licensed Practical Nurse (LPN) to join the Hospice LPN Team Resource in Massachusetts. Under the supervision of the RN Case Manager, you’ll perform professional nursing care and contribute to assessment, planning and evaluation of patient care.
You’ll perform nursing visits, apply palliative care principles, report changes to physicians, and join weekend/holiday rotations.

The Hospice LPN Team Resource performs professional nursing care, functions as a member of the hospice team and provides input into the assessment, planning, and evaluation of nursing services. Under the direction and supervision of the RN Case Manager - Hospice, the LPN is accountable for the quality of nursing care he or she provides and assumes responsibility for the provision of nursing care in accordance with the established plan of care for assigned patients.

Pay Range: $32.00 - $43.07

Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
