Job summary

Beth Israel Lahey Health at Home is seeking a dedicated Hospice RN Case Manager to plan and deliver patient-centered hospice care using the nursing process. You will assess patients and families, develop care plans, manage symptoms, and educate on medications and the dying process.

Requirements include an Associate’s Degree or BSN, Massachusetts RN license, BLS certification, and at least 1 year of direct nursing experience (home health preferred).

Qualifications

  • Active Massachusetts RN license required.
  • BLS certification required.
  • At least 1 year direct care nursing experience; home health/public health preferred.
  • EPIC experience preferred.

Responsibilities

  • Complete and document an initial assessment of patient and family to determine hospice care needs.
  • Perform a complete physical assessment of patient and history of current and previous illness(es).
  • Provide information to the patient and family regarding the dying process and expectations.
  • Develop a patient-specific plan of care with goals based on nursing diagnosis, focusing on pain and symptom management.
  • Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency.
  • Document services using problem-oriented records.
  • Instruct and supervise care provided by Hospice Aides; supervise every two weeks, as needed.
  • Participate in bi-weekly hospice case conferences or as required.
