Job summary

Reliable Home Care Providers, Inc. is hiring a Russian-speaking Home Health RN to deliver skilled nursing visits in the O’Hare area. Build your own schedule, manage your caseload, and enjoy one‑on‑one care with the autonomy of field nursing backed by a top-performing agency.

You will provide skilled visits, coordinate with physicians and caregivers, supervise nursing staff, and document care in WellSky EMR. Valid IL RN license, CPR/BLS, and bilingual proficiency are required.

Qualifications

• Current Illinois RN license in good standing
• Fluent in Russian and English (required)
• Current CPR/BLS certification
• Valid driver's license with reliable vehicle and auto insurance for travel between patient homes
• Home health experience and OASIS familiarity preferred
• WellSky Home Health EMR experience is a plus
• Strong assessment, problem‑solving, and communication skills
• Ability to perform the essential functions of field nursing, with or without accommodation

Responsibilities

• Perform the nursing process: assessment, planning, implementation, and evaluation, including OASIS documentation
• Provide skilled nursing visits and oversee patient care from admission through discharge
• Develop and update the plan of care based on clinical findings
• Advocate for patients and coordinate with physicians, referrals, caregivers, and facility staff
• Supervise nursing staff (LPNs and HHAs)
• Coordinate care across the multidisciplinary team (PT, OT, ST, MSW, HHAs, office staff)
• Complete all skilled nursing documentation timely and accurately in the EMR
