Job summary

Reliable Home Care Providers, Inc. in Lake County, IL is seeking a dedicated Home Health RN to deliver skilled nursing visits in patients' homes.

Build your own schedule with self-scheduling, manage your own caseload, and provide true one-on-one care with the autonomy of field nursing. You will perform assessments, develop and update care plans, coordinate with physicians and care teams, and document in WellSky EMR.

Qualifications

• Active Illinois RN license required.
• CPR/BLS certification required.
• Valid driver's license and reliable vehicle for home visits.
• Home health experience and OASIS familiarity preferred.

Responsibilities

• Perform nursing assessments and document using OASIS.
• Provide skilled nursing visits and manage patient care from admission to discharge.
• Develop, update, and coordinate the care plan.
• Advocate for patients and communicate with physicians and caregivers.
