Qualifications

• Current, active Illinois RN license in good standing.
• Current CPR/BLS certification.
• Prior nursing experience; home health experience preferred.
• Valid driver's license and reliable vehicle required.

• Provide skilled nursing visits and oversee patients.
• Develop and update the plan of care based on clinical findings.
• Coordinate care with physicians and the multidisciplinary team.
