Community Health Resources (CHR) is seeking a dedicated Recovery Support Specialist for Rural Pathways in Danielson, CT. This full-time role focuses on clinical assessments, treatment planning, and recovery-focused services to individuals with mental health and substance use disorders.

The position emphasizes Motivational Interventions, crisis planning, and connecting clients with housing, transportation, and community supports within a rural network.

Qualifications

• High School Diploma required or equivalent.
• Recovery Support Specialist Certification required or must obtain within one year of hire.
• Valid Driver’s License and access to a personal vehicle are required.

Responsibilities

• Conducts clinical and risk assessments and LOCUS.
• Develops person-centered treatment plans and documents progress.
• Use Motivational Interventions to support client goals.
• Provide psycho-educational rehab services for recovery from mental health and substance use disorders.
• Assist clients in linking with community supports and housing resources.
• Create crisis plans and respond to crises with staff as needed.
