Job summary

Community Health Resources (CHR) is seeking an APRN for Rural Pathways in Danielson, CT. This full-time role supports MOUD and behavioral health services across rural northeastern Connecticut.

The APRN will provide psychiatric evaluations, medication management, and crisis planning under physician supervision, with a focus on rapid access to care and integrated treatment for co-occurring disorders. L icensure and 5+ years in MH/addiction preferred.

Qualifications

• CT APRN license and RN license; CT CSP license; and DEA number.
• 5 years’ experience in adult or child mental health.
• Community mental health and/or addiction treatment experience preferred.

Responsibilities

• Prescribe medications under agency policies and procedures under the direction and supervision of the agency MD.
• Direct Treatment Services.
• Provide direct clinical services to clients, including psychiatric evaluations, diagnostic assessments, medication management, and crisis planning.
• Provide addiction treatment including MOUD initiation and titration, and evaluation of comorbidities.
• Orders and reviews lab work, EKGs, or outside medical records as appropriate.
• Administer medications as needed.
• Prioritize same-day psychiatric evaluations or addiction medicine consults when available.
• Maintain relationships with community health organizations for coordinated care.
• Provide staff training and consultation.
• Develop programs and psycho-educational interventions for clients.
• Participate in case conferences and team meetings.
• Makers comprehensive bio-psycho-social assessments and treatment focus.
