Coastal Home Health & Hospice is seeking a Licensed Practical Nurse to administer skilled nursing care in patients' homes across rural Oregon under the direction of the Clinical Director.

The role requires Oregon LPN licensure and BLS, with 1 year of prior home health or hospice experience preferred. You will work with the RN to implement care plans, document care, and communicate with families.

Qualificações

• Oregon-licensed LPN with current license.
• BLS certification required.
• Experience in home health, hospice, or facility setting preferred.
• Ability to work with limited supervision and in home environments.

Responsabilidades

• Understands and adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
• Observes signs and symptoms and reports to physician and RN.
• Teaches and counsels patient and family regarding care needs.
