Job summary

Coastal Home Health & Hospice is seeking a Hospice RN Case Manager to provide direct skilled nursing care in patients' homes, coordinating with the interdisciplinary team to implement and adjust plans of care.

The role requires an Oregon RN license, BLS, and 1 year of nursing in home health, hospice, or a facility setting (3 years preferred). Travel between Brookings, Gold Beach and Bandon is expected; on-call rotation may be required.

Qualifications

• Graduate of an approved school of professional nursing.
• Current unencumbered RN nursing license to practice in Oregon.
• Current BLS certification or ability to obtain prior to hire.

Responsibilities

• Provide professional nursing in the hospice program at patients' homes.
• Lead primary case management for individual patients/families with care plan updates.
• Delegate tasks to home health aides per policy and supervise accordingly.
• Maintain timely documentation including assessments, care plans and notes.
• Collaborate with the Interdisciplinary Team and participate in care conferences.
