Job summary

CivicMinds, Inc. is seeking a compassionate LPN to deliver skilled nursing care to patients in their homes under the supervision of an RN, following physician orders and individualized care plans.

The role requires a valid Oregon LPN license, current BLS certification, and at least one year of home health or hospice experience; three years preferred. Travel throughout the service area is required, with opportunities to make a meaningful difference in rural healthcare while maintaining accurate

Qualifications

• Graduate of an approved practical nursing program with an active Oregon LPN license.
• Current BLS certification.
• At least one year of nursing experience in home health, hospice, or a healthcare facility; three years preferred.
• Knowledge of nursing procedures, medications, pain management, and EMR.
• Strong communication skills and ability to work independently.
• Valid Oregon driver's license, vehicle insurance, and a good driving record.

Responsibilities

• Provide nursing treatments and implement patient care plans.
• Monitor patients' conditions and report changes to the supervising RN and physician.
• Educate patients and families on care needs and treatment plans.
• Maintain accurate clinical notes and patient records.
• Assist RNs with specialized procedures and participate in patient care conferences.
• Follow nursing standards, confidentiality policies, and applicable regulations.
