Job summary

Riverside Home Health & Hospice in Southern Oregon seeks a Licensed Practical Nurse (LPN) to provide in-home patient care across Medford, Grants Pass and surrounding communities. The role emphasizes patient-centered care, independence, and collaboration with RNs and the care team.

Qualifications

• Completion of an accredited practical nursing program.
• Current Oregon LPN license.
• Three years nursing experience preferred.
• Home health or medical-surgical experience preferred.
• Strong verbal and written communication skills.
• Strong organizational skills and reliable transportation.

Responsibilities

• Provides direct patient care under the supervision of a registered nurse.
• Implements plan of care and documents accordingly.
• Collects assessment data and reports to RN and physicians.
• Participates in coordination of home health services with the care team.
• Uses equipment and supplies efficiently.
• Performs other duties as assigned by the RN.
