Job summary

Mercy is seeking a Rural Health Transformation Program Coordinator to lead an integrated hub, oversee Navigator staff, and drive program performance. You will implement governance support, coordinate across counties, and ensure alignment with Mission leadership and Hub Boards.

Responsibilities include providing operational leadership, stakeholder facilitation, and building partnerships with regional networks to advance rural health initiatives and program outcomes.

Qualifications

• Bachelor’s degree required in Public Health, Social Work, Healthcare Administration, Community Health, or related field.
• Three to five years of experience in community health, program coordination, healthcare outreach, or related field.
• Demonstrated leadership, supervisory, or team coordination experience.
• Strong communication, facilitation, organizational, and project management skills.

Minimum Qualifications

• Strong communication, facilitation, organizational, and project management skills.

Preferred Qualifications

• Master’s degree preferred.
• Experience managing multi-site or community-based programs.
• Familiarity with rural health initiatives, government-funded programs, and community partnerships.
• Completion of a Community Health Worker training program (such as Partners in Care) preferred at hire.
• Ability to obtain Community Health Worker credentialing preferred.
• Grant writing, grant administration, or grant reporting experience preferred.
