Hopscotch Primary Care seeks an Associate Outreach Leader to advance patient growth and community engagement across clinics and territories. You will build relationships, coordinate events, and guide prospective patients through enrollment in a fast-paced, mission-driven environment.

You’ll work with Outreach Leaders, clinic teams, marketing, and patient engagement to maximize community presence and drive growth while ensuring a high standard of care access for seniors and communities served.

Qualifications

• High school diploma or equivalent required.
• Valid driver's license and ability to travel locally.
• U.S. work authorization.
• Experience in healthcare outreach or community engagement preferred.
• CRM and basic Microsoft Office familiarity.

Responsabilités

• Build relationships with community organizations and stakeholders.
• Coordinate outreach events, health fairs, and clinic open houses.
• Educate prospective patients about Hopscotch services and enrollment steps.
• Track activities and follow up to improve patient conversion.
• Collaborate with clinic teams, marketing, and patient engagement to support growth.
