Job summary

Hopscotch Primary Care in Shelby, NC is seeking an Associate Outreach Leader to support patient growth and community engagement across clinics. You will coordinate events, educate prospective patients about Hopscotch services, and help guide enrollment.

The role works with Outreach Leaders, clinic teams, marketing, and patient engagement to maximize community presence. Ideal candidates are relationship builders who enjoy working with seniors and local organizations in a fast-paced mission-driven

Qualifications

High school diploma or equivalent required.
Valid driver's license and U.S. work authorization.
Ability to travel locally within assigned territory.
Preferred: 1–3 years in healthcare outreach, community engagement, sales support, event coordination, or customer service.

Responsibilities

Build and maintain relationships with grassroots community organizations such as senior centers, housing communities, faith-based groups, nonprofits, and local civic groups.
Represent Hopscotch Primary Care professionally within the community and serve as a trusted contact for prospective patients.
Educate community members about Hopscotch services and the value-based care model; conduct outreach to increase awareness and engagement.
Coordinate outreach calendars, events, and collaboration with internal teams to maximize coverage and minimize duplication.
