Job summary

The RHTP Navigator at Ozarks Community Hospital, Sparta Clinic, coordinates care across hospitals, clinics, EMS, and community partners in rural Missouri. You will screen, refer, and support residents to access primary care, behavioral health, EPSDT, and social services while navigating barriers to care.

Qualifications

• High school diploma or equivalent with relevant community health experience.
• Experience with rural/high-need populations and barriers.
• Ability to document with digital systems and manage referrals.
• Valid driver's license and ability to travel within Hub area.

Responsibilities

• Conduct standardized screening and intake; document findings and referrals in CIE and Hub systems.
• Manage closed-loop referrals and warm handoffs across Hub partners; re-engage residents when needed.
• Engage Medicaid members and high-need residents through calls, field visits, and outreach; provide health education and navigation.
• Help residents access Hub-supported services including primary care, behavioral health, and EPSDT.
• Identify and resolve barriers to care; escalate as needed for program review.
• Maintain assigned caseload and follow-up until services are completed or transitioned.
