Qualifications

• High school diploma or equivalent with relevant community health experience.
• Experience with care coordination and patient navigation is preferred.
• Ability to travel routinely within a multi-county Hub service area.

Responsibilities

• Conduct screening to identify clinical, behavioral, and social needs and document in CIE.
• Manage closed-loop referrals and warm handoffs across partners.
• Engage Medicaid/high-need residents and provide health education and navigation.
• Assist residents to access Hub-supported services including primary care and behavioral health.
• Identify and resolve barriers to care; escalate as needed.
• Own assigned caseload and maintain structured follow-up until completion.
