Job summary

The Purchase District Health Department seeks a dedicated Community Health Worker to join the Rural Community Hubs for Chronic Care Innovation program. You will connect residents with health and social resources, support care coordination, and drive community engagement across five counties in the Purchase Area.

Qualifications include CHW certification (or eligibility), rural health experience, and excellent communication.

Qualifications

• Community Health Worker certification or eligibility preferred.
• Experience in rural communities and hub-based programs preferred.
• Ability to document activities and support continuous improvement.

Responsibilities

• Build relationships with residents and promote Hub services.
• Help residents access health, social, and community resources.
• Coordinate care across providers and community organizations.
• Gather resident input to inform Hub planning and initiatives.
• Document outreach activities, referrals, and follow-ups.
