Job summary

Heart Med LLC in Skokie, IL seeks a detail-oriented Registered Nurse to support our Remote Patient Monitoring program from our office. You will review patient data from RPM devices, identify abnormal trends, and escalate as needed while documenting actions in the EHR.

The ideal candidate has an Illinois RN license, experience with electronic health records, and strong communication skills to coordinate with care teams and providers in a fast-paced, compliant healthcare setting.

Qualifications

• Current, unrestricted Illinois RN license or NP? (ensure IL recognition)
• Graduation from an accredited nursing program.
• Experience using electronic health records and RPM-related systems.
• Strong clinical judgment and attention to detail.

Responsibilities

• Review and interpret RPM data per clinical protocols.
• Monitor readings, trends, alerts, and schedule adherence.
• Identify abnormal, missing, or inaccurate readings.
• Triage and escalate findings to supervising provider as needed.
• Notify provider or care team for further evaluation.
