The Bridge Project is recruiting a full-time Rough Sleeper Recovery Coordinator to join the New Vision Bradford partnership and support service users from entry through recovery. The role requires travelling across the district, with a valid driving licence, vehicle access and work insurance.

You will work within a multi-disciplinary team to deliver high-quality drug and alcohol treatment and outreach for rough sleepers or those at risk.

Qualificações

• 2 years’ full-time experience working with people with alcohol and/or drug problems within a 4-year period.
• Experience of case management and delivering care coordination in drug and alcohol or related field.

Responsabilidades

• Coordinate tailor-made packages of care for an assigned group of service users.
• Provide comprehensive assessments and recovery planning.
• Deliver one-to-one key work and recognised psychosocial interventions.
• Support harm minimisation and treatment engagement across the recovery journey.
• Lead outreach to connect with rough sleepers or those at risk of rough sleeping.
