Job summary

The Bridge Project is seeking Recovery Coordinators for its Treatment Team at New Vision Bradford. We are hiring full-time and part-time posts to deliver high-quality drug and alcohol treatment, supporting service users from entry to recovery.

You will act as the main care coordinator for an assigned group, conducting screenings, asset mapping, comprehensive assessments, recovery planning and care reviews in a multi-disciplinary team.

Qualifications

• Two years’ full-time experience in alcohol/drug services or related field within the last four years.
• Experience in case management and delivering care coordination.
• Excellent communication and highly organised approach.

Responsibilities

• Act as the main care coordinator for an assigned group of service users.
• Provide screening, asset mapping, and comprehensive assessments.
• Lead recovery planning and reviews of care within a multi-disciplinary team.
