Qualifications

Responsibilities

• Provide clinical pharmacy services on rotation to acute medical, surgical, emergency, and critical care units under supervision of Lead Specialist Pharmacists.
• Advise prescribers on doses, side effects, and treatment options; communicate complex pharmaceutical recommendations to medical and nursing staff.
• Interview newly admitted patients to construct an accurate medication history and review admission details and prescriptions.
• Assist with pharmaceutical care plans, monitor patient progress, and intervene to optimise therapy with minimal toxicity.
• Provision of professional dispensary services: scrutinise and ensure accuracy of prescriptions dispensed; ensure cost-effectiveness and formulary compliance.
• Provide cancer service support including chemotherapy prescription checks and GMP-compliant pre-check and release in aseptic units; assist with blood monitoring and discharge information.
