The role includes risk management, medicines information and dose optimization, with involvement in chemotherapy checking and discharge planning.

You will communicate complex recommendations to medical and nursing staff, interview new admissions for medication histories and ensure compliance with medicines

Responsibilities

• Advise medical, nursing and technical staff on safe medicines management and disposal.
• Counsel patients and provide medicines information, including for those with complex needs.
• Rotationally check prescriptions for accuracy, dosing and formulary compliance.
• Query availability, location and cost of medicines using the Pharmacy Computer System and dispense accordingly.
• Process discharge prescriptions using the Electronic Discharge System.
• Check chemotherapy prescriptions on a rotational basis to ensure appropriate dosing and safety.
• Provide day-to-day clinical supervision for junior staff and students.
• Support training and CPD and contribute to service development.
• Assist in security of medicines and respond to emergency pharmaceutical duties.
