Job summary

Qualifications

• Current Registered Nurse in the State of Pennsylvania in good standing.
• 1+ years of experience in Minimum Data Set assessments and reporting.

Responsibilities

• Conduct comprehensive assessments of residents to identify medical, social, and psychological needs.
• Develop and implement individualized care plans that address residents' needs and goals.
• Coordinate with physicians, therapists, and social workers to ensure comprehensive care.
• Ensure smooth transitions between hospital, rehabilitation, and long-term care settings.
• Monitor residents' progress and adjust care plans as needed.
• Maintain accurate records of assessments, care plans, and outcomes.
• Participate in quality improvement initiatives to ensure regulatory compliance.
