Job summary

Keswick, located in Baltimore, MD, seeks an RNAC/MDS Coordinator to implement and evaluate the MDS program and support care planning in alignment with Keswick’s philosophy.

The role ensures documentation supports reimbursement and regulatory compliance while collaborating with a compassionate, award-winning team dedicated to improving aging experiences.

Qualifications

  • Registered Nurse (RN) licensed and in good standing with MBON.
  • Current AHA HCP BLS certification.
  • Three years of experience as an MDS Coordinator/RNAC.
  • Resident Assessment Coordinator Certified (RAC-CT) must obtain within 6 months of hire.

Responsibilities

  • Meets with residents in-person to collect information for the Resident Assessment Instrument (RAI) and assess health status.
  • Analyzes data to inform nursing diagnoses, plan interventions, and evaluate care needs.
  • Communicates and consults with other health team members.
  • Completes comprehensive resident assessment (initial, quarterly, annual) and ensures documentation supports reimbursement.
  • Completes MDS and MMDS per policy and transmits to the State.
  • Contributes to the development of an updated plan of care based on identified problems.
  • Reviews documentation of direct care staff and provides education on MDS requirements.
  • Keeps the Director of Clinical Reimbursement informed of major issues and reports concerns as appropriate.
  • Educates care-delivery team on MDS information and procedures.
  • Complies with regulatory requirements and participates in surveys and inspections.
  • Prepares data and attends triple-check meetings as needed.

What is required for the RNAC/MDS Coordinator position?

  • Registered Nurse (RN) licensed and in good standing with the MBON

What will I do as an RNAC/RN MDS Coordinator with Keswick?

  • Meets with resident/patient in-person to collect information (initial and ongoing) for the Resident Assessment Instrument (RAI) and provides a focused assessment of the resident/patient health status that will be used in addressing anticipated changes in resident/patient condition as well as emergent changes in health status; recognizes alterations to previous conditions; synthesizes the biological, psychological, spiritual, and social aspects of the resident/patient condition; evaluates the impact of nursing care. Attends clinical meetings.
  • Uses this analysis to make decisions and nursing diagnoses, plan nursing interventions, and evaluate the need for different interventions. Recommendations are shared with the Unit Manager and/or DON as appropriate.
  • Communicates and consults with other health team members
  • Completes comprehensive resident assessment (initial, quarterly, annual); ensures that documentation meets requirements of Keswick and regulatory agencies and is sufficient to substantiate reimbursement for Keswick services
  • Completes MDS and MMDS according to established policies and procedures; locks and transmits to the State
  • Contributes to the development of a plan of care based on identified resident/patient problems and needs; evaluates resident/patient response to the plan of care and recommends appropriate changes
  • Reviews documentation of direct care staff and provides education to ensure documentation meets requirements of Keswick and regulatory agencies.
  • Keeps the Director of Clinical Reimbursement (DCR) informed of major or recurring problems; communicates any operational or patient care issues to the Unit Manager and/or DON as appropriate.
  • Educates other members of the care-delivery team on MDS information and procedures.
  • Complies with regulatory and legislative requirements; participates as needed in federal, state and local surveys and inspections; follows any plans of correction; reports abuse, neglect or violence according to established procedures; addresses complaints from patients, families, or physicians or refers complaints to the Unit Manager.
  • Prepares data and/or attends triple-check meetings as needed.
