Job summary

Keswick in Baltimore, MD is hiring an LPN MDS Coordinator to join our not-for-profit aging services team. You will meet with residents in person to collect information for the Resident Assessment Instrument and perform focused assessments that support care planning and regulatory reimbursement.

The role requires an accredited nursing program graduate, LPN license in good standing, BLS, at least three years of MDS experience, and RAC-CT or willingness to obtain.

Qualifications

  • Must be a graduate of an accredited nursing program.
  • Licensed Practical Nurse (LPN) in good standing with MBON.
  • AANAC-RAC-CT certification or willing to obtain within 6 months of hire.

Responsibilities

  • Meets with residents to collect information for the Resident Assessment Instrument (RAI) and provides focused assessments to inform care planning.
  • Makes nursing interventions recommendations and communicates with health team members as needed.
  • Completes focused resident assessments (initial, quarterly, annual) and ensures documentation supports reimbursement.
  • Completes MDS and MMDS per policy and contributes to the plan of care based on identified needs.
  • Keeps leadership informed of problems and communicates operational or patient care issues to the Unit Manager or DON as appropriate.
  • Educates care-delivery staff on MDS information and regulatory requirements.

What will I do as an LPN MDS Coordinator with Keswick?
  • Meets with resident/patient in person to collect information (initial and ongoing) for the Resident Assessment Instrument (RAI) and provides a focused assessment of the resident/patient health status that will be used in addressing anticipated changes in resident/patient condition as well as emergent changes in health status; recognizes alterations to previous conditions; synthesizes the biological, psychological, spiritual, and social aspects of the resident/patient condition; and assists in evaluating the impact of nursing care. Attends clinical meetings.
  • Uses this information to recommend nursing interventions and the possible need for different interventions and assesses the need to communicate and consult with other health team members; submits recommendations to Sr. RNAC and/or Director of Clinical Reimbursement (DCR) for review.
  • Completes a focused resident assessment (initial, quarterly, annual); ensures that documentation meets requirements of Keswick and regulatory agencies and is sufficient to substantiate reimbursement for Keswick services.
  • Completes MDS and MMDS according to established policies and procedures.
  • Contributes to the development of the plan of care based on identified resident/patient problems and needs; observes resident/patient response to the plan of care and recommends appropriate changes.
  • Keeps Sr. RNAC and/or Director of Clinical Reimbursement (DCR) informed of major or recurring problems; communicates any operational or patient care issues to the Unit Manager/DON as appropriate.
  • Reviews documentation of direct care staff and provides education to ensure documentation meets the requirements of Keswick and regulatory agencies.
  • Educates other members of the care-delivery team on MDS information and procedures.
  • Complies with regulatory and legislative requirements; participates as needed in federal, state, and local surveys and inspections; follows any plans of correction; reports abuse, neglect, or violence according to established procedures; addresses complaints from patients, families, or physicians or refers complaints to the Unit Manager.
What is required for the LPN MDS Coordinator position?
  • Graduation from an accredited school of Nursing
  • Licensed Practical Nurse (LPN) current and in good standing with the MBON
  • Current AHA HCP BLS certification
  • Three years of experience working with MDS
  • AANAC-RAC-CT Certified (or willing to obtain within 6 months of hire)
