Job summary

Yale NewHaven Health is seeking a Home Infusion Registered Nurse to deliver high-quality infusion therapy in patients' homes or ambulatory settings. The role includes patient assessments, managing IV lines, and educating patients and caregivers.

Requires RN with CT licensure and a minimum of three years IV experience; BSN preferred. CRNI certification is a plus. Excellent communication and computer skills are essential; weekend rotation may be required.

Qualifications

Responsibilities

  • Provide nursing services at home or alternate sites for intravenous, subcutaneous, oral medications in accordance with policies.
  • Collaborate with physicians, pharmacy team, and care team to coordinate patient care.
  • Educate patients and caregivers on care needs and self-administration when applicable.
  • Follow infection control measures and maintain confidentiality of patient information.
  • Maintain accurate nursing records and participate in team meetings and policy development.

The Home Infusion RN is a member of the YNHH Home Infusion Therapy team. Responsible for the delivery of high-quality infusion therapy and nursing services in the home or alternate settings (i.e.; Ambulatory Infusion Suites).This includes performing patient assessments, managing various vascular access devices, and educating patients and/or caregivers to ensure safe and effective treatment according to physician's plan of care. Provides education to patient/and/or caregiver on proper handling, storage and administration of infusion medications as well as signs and symptoms of access device complications.

Responsibilities
  • 1.Provides nursing services at home and/or alternate sites for intravenous, subcutaneous, oral, etc. medication administration for YNHHS Home Infusion Pharmacy patients in compliance with YNNHS policies.
  • 2.Works in collaboration, coordinates patient care, and maintains communication with the physician, pharmacy team, and other members of the collaborative treatment team.
  • 3.Teaches patients and/or family/caregivers about the patient's care needs (including self-administration when applicable) and disease prevention
  • 4.Follows and adheres to infection control measures and standards for home care nursing and IV nursing services
  • 5.Maintains accurate and timely clinical nursing records according to YNHHS Home Infusion Pharmacy policies
  • 6.Adheres to ethical nursing standards and maintains the confidentiality of patients and their personal information at all times
  • 7.Perform other duties as directed by home infusion leadership to support the needs of the department and represents the home infusion pharmacy in a professional and positive manner.
  • 8.Stay abreast of developments in the field of infusion therapy and comply with all YNHHS policies and directives for the fulfillment of the YNHHS objectives
  • 9.Participates in staff meetings, in-service programs, and serves on committees as requested by the Manager of Home Infusion
  • 10.Plays a significant role in the development, evaluation and implementation of standards of practice and related policies and procedures, ensures compliance with regulatory and accreditation standards relating to nursing
Qualifications
EDUCATION

Nursing diploma or associate degree in nursing. Bachelor's degree in nursing preferred. CRNI certification a plus.

EXPERIENCE

Three years of IV related experience. Previous experience in the home care of out patient infusion setting is desirable. Case Management experience a plus.

LICENSURE

Registered Nurse with valid (or eligible) Connecticut licensure required.

SPECIAL SKILLS

Experienced in the care of peripheral and central lines. The ability to troubleshoot potential problems that may arise in dealing with IV therapy. Knowledge of medications, indications, dosage ranges, side effects, and potential toxicities. Ability to work independently. Must have excellent customer service skills (both verbal and written) and have effective interpersonal skills. Proficient computer skills.

PHYSICAL DEMAND

Must be capable of lifting 20 lbs, standing, walking, and bending. Weekend rotation based on operational need.
