Responsibilities

• Assess residents' health care needs and translate into an interdisciplinary care plan
• Communicate the plan to care providers to ensure continuity of care
• Administer medications and treatments per standards of practice
• Document care and treatment accurately in records and ensure compliance with requirements
• Support MDS/MMQ processes and contribute to center quality and compliance
• Provide education to residents about their care plans and health issues
• Lead nursing staff in safe, effective delivery of care and ensure respectful interactions
