Responsibilities

Job Summary

The RN in Home and Community Based Services provides nursing and personal care to patients in their homes, as prescribed by physicians and compliant with laws, regulations, and agency policies. The RN coordinates care plans with healthcare professionals, patients, families, and community resources to ensure continuity of care.

Specific Duties

• Provide high-quality clinical services within scope and infection control standards, following care plans from admission to discharge.
• Conduct clinical nursing assessments per federal and state requirements.
• Verify patient eligibility and medical necessity for services.
• Develop and update individualized care plans with community providers.
• Ensure care plan meets patient needs, revising as needed with physician approval.

Education and Experience

Experience Desired

• Minimum one year RN experience preferred.

License Requirements

• Current RN licensure in practice state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

State-Specific Requirements

• LA: At least one year of clinical RN experience; may be waived for RNs with recent LPN clinical experience. RN licensure must be unrestricted.
