Responsibilities

Job Summary

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, complying with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals to ensure continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, according to the care plan, and in coordination with the care team from admission to discharge.
• Completes clinical nursing assessments as required by federal and state program requirements and payers.
• Ensures patient eligibility and medical necessity for services as defined by payer and agency policy.
• Develops and revises individualized care and service plans in accordance with federal and state requirements, collaborating with community providers.
• Ensures the plan of care's frequency and duration meet patient needs, initiating revisions with physician approval as needed.

Education and Experience

Experience Desired

• Minimum of one year experience as an RN preferred.

License Requirements

• Current RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

State Specific Requirements

• LA: At least one year of clinical experience as an RN. This may be waived for RNs with recent clinical experience as an LPN. RN licensure must be unrestricted.
