Qualifications

• Minimum 3 years of clinical practice/experience.
• 1 year with one to two years of case management/care coordination experience preferred.
• RN licensure and current BLS/ACLS as required.

Responsibilities

• Coordinate and oversee a team across acute to long-term patient stages.
• Collaborate with discharge planning and care managers to optimize disposition planning.
• Identify barriers to timely discharge and coordinate with healthcare team and payors.
