Qualifications

• Experience with triage in clinic, urgent care, or physician office settings.
• Ability to uphold safety, infection control and quality standards.

Responsibilities

• Provide skilled nursing services under physician or nurse practitioner supervision.
• Collect and analyze patient data to determine diagnoses and outcomes.
• Develop and implement a care plan to achieve expected outcomes.
• Coordinate interventions and evaluate progress with the care team.
• Educate patients and families on health conditions and care plans.
