Qualificações

• Associate Degree in Nursing required; Bachelor’s degree preferred.
• Valid NYS RN license required.
• Minimum 1 year RN experience in long-term care required.
• 3–5 years of long-term care with supervisory experience preferred.
• Strong knowledge of resident assessments, care planning, and LTC regulations.
• Leadership experience with ability to supervise, coach, and mentor staff.

Responsabilidades

• Oversee the clinical care and daily operations of an assigned nursing unit.
• Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff.
• Conduct resident assessments and ensure care plans are implemented, updated, and followed appropriately.
• Monitor resident conditions and communicate changes with providers, families, and team members.
• Review provider orders, lab results, incident reports, and quality indicators.
• Participate in care plan meetings, family meetings, QAPI activities, and rounds.
• Coordinate admissions, discharges, transfers, and resident services.
• Ensure accurate documentation, medication management, and regulatory compliance.
• Support staff education, orientation, coaching, and performance management.
