Job summary

Qualifications

• Active nursing license in NY.
• Experience in medical-surgical nursing preferred.
• Strong communication skills, both verbal and written.
• Empathy and patience when engaging with patients and families.
• Ability to work independently within established protocols.

Responsibilities

• Conduct comprehensive UAS-NY assessments to determine eligibility and care needs.
• Collaborate with interdisciplinary teams to promote PACE program and educate sources.
• Review and maintain accurate EMR documentation, addressing health changes.
• Engage in quality improvement initiatives following CMS/PACE guidelines.
