Job summary

Live Well Homecare is seeking an RN Triage – OBGYN to perform telephone and in-person patient assessments in a fast-paced healthcare setting. You will direct patients to appropriate care levels and assist with immunizations and venipuncture as needed.

The role requires Illinois RN licensure and BLS CPR certification, with preferred Bachelor’s degree and 1 year of triage experience. You’ll work across multiple locations, collaborating to deliver high-quality community care.

Qualifications

Responsibilities

• Review patient voicemails and communicate with patients via phone or MyChart, offering medical advice or directing to emergency or urgent care as needed.
• Conduct nursing assessments and teach-back to ensure understanding of care plans.
• Identify high-risk symptoms and refer to emergency services when appropriate.
• Schedule appointments for non-emergency care.
• Assist with patient intake, record vital signs, prepare patients for examinations, and perform immunizations and diagnostic procedures.

Key Responsibilities

• Review patient voicemails and communicate with patients via phone or MyChart, offering medical advice or directing to emergency or urgent care as needed.
• Conduct nursing assessments, intervene based on patient needs, and ensure understanding through teach-back methods.
• Identify high-risk symptoms such as chest pain or severe headaches and immediate referral to emergency services.
• Schedule appointments for patients requiring non-emergency care.
• Assist with patient intake, record vital signs, prepare patients for examinations, and perform immunizations and diagnostic procedures (e.g., venipuncture, specimen collection).

Qualifications & Skills

• Current Illinois RN licensure and BLS CPR certification are required.
• Preferred: Bachelor’s degree in Science and 1 year of telephone triage experience in a medical setting.
