Job summary

Responsibilities

• Assess the patient's physiologic health status.
• Conduct patient interviews and explain policies and procedures.
• Assess GI, CV, respiratory, renal, and neuro health.
• Determine mobility, sensory deficits, prostheses, and skin condition.
• Assess pain level and manage pain.
• Document patient health status and plan of care.
• Assess psychosocial health and patient expectations.
• Develop and implement the plan of care, prioritizing outcomes.
• Administer medications safely.
• Educate patients and families about care.

What You'll Do

• Assess the patient's physiologic health status.
• Conduct patient interviews and explain policies and procedures.
• Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
• Determine mobility, sensory deficits, prostheses use, and skin condition.
• Assess level of pain and pain management.
• Communicate and document patient's physiologic health status and plan of care.
• Assess patient's psychosocial health status.
• Elicit perception of medical/nursing care and expectation of care.
• Identify patient outcomes and develop criteria for measurement.
• Formulate the plan of care and establish priorities to achieve expected outcomes.
• Implement the plan of care and maintain constant vigilance over patient care.
• Evaluate care provided for patient outcomes and perform variance analysis.
• Collaborate with care team members in planning and carrying out treatment regimens.
• Administer medications correctly and safely.
• Participate in patient and family education.

Requirements

Education:

• Associate's Degree in Nursing required, Bachelor's Degree in Nursing preferred.

Experience:

• Minimum 1-2 years of professional nursing experience in the related specialty.

Licenses & Certifications:

• Current license to practice as a Registered Professional Nurse in New York State required.
• BLS certification required.
