Job summary

• Communicate and document patient’s physiologic health status and plan of care.
• Assess patient’s psychosocial health status.
• Elicit perception of medical/nursing care and expectations.
• Develop criteria for measurement of patient outcomes.Formulate the plan of care and establish priorities.
• Implement the plan of care and maintain patient safety.
