Job summary

Qualifications

• Experience coordinating care with transplant clinics and post-transplant care is essential.

Responsibilities

• Coordinate care with transplant clinics and community referrals.
• Assist with outpatient treatments/infusions and follow-up hospital visits.
• Review candidates and facilitate testing to avoid deactivation; monitor post-transplant labs.
• Escalate issues to the multidisciplinary team and communicate changes with the MD.
• Maintain documentation for transplant candidacy and post-transplant follow-up.

Unit Details: Post-Kidney transplant coordinator will have experience with coordinating care with transplant clinics, community referrals, outpt treatments/ infusions, follow up hospital visits, Reviews candidates and assists facilitating testing to avoid deactivation, Monitors pts to identify and resolve clinical issues, monitoring post transplant labs, escalates to multidisciplinary team, works with MD to communicate output immunosuppression and other med changes, maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.

Healthcare Professionals must satisfy any required credentialing items before traveling to a strike. USN will prioritize booking travel arrangements for Healthcare Professionals with a complete file. If you arrive on-site with missing or outstanding items, you may not be eligible to work the strike.
