Wellstar Health System is seeking a transplant coordinator RN to manage coordination of care for transplant patients and living donors. You will educate patients and families, coordinate across multidisciplinary teams, and ensure timely documentation and compliance with HIPAA and program policies.

You will work with a qualified team to support pre-operative, inpatient, discharge, and long-term follow-up phases, driving quality improvement and patient-centered care within the Georgia transplant

Formación

Responsabilidades

• Facilitate comprehensive patient care coordination across medical, surgical, financial, psychosocial, nutrition and pharmacy domains.
• Support education for patients and families about the transplant process and ensure clear, ongoing communication.
• Act as case manager for transplant patients and living donors through all phases (pre-op to long-term follow-up).
• Document care accurately and timely; maintain HIPAA compliance and EMR updates.
• Engage in quality monitoring and process improvement within the transplant program.
