Job summary

Wellstar Health System in Georgia is seeking a transplant coordinator RN to coordinate care for transplant patients, living donors, and their families. You will educate, support, and coordinate across the continuum of care, contributing to program development and quality improvement.

You will work as part of a multidisciplinary team, manage documentation, and ensure HIPAA compliance while guiding patients through pre-, peri-, and post-transplant stages.

Qualifications

• RN licensure in good standing is required.
• BLS Instructor or BLS certification required.
• RN-Compact or Single State RN license required.

Responsibilities

• Facilitates medical, surgical, financial, psychosocial, nutrition and pharmaceutical evaluation and ongoing care needs for transplant candidates/recipients and living donors.
• Maintains open communication with patients, families and the multidisciplinary team; ensures plan of care is known to all team members.
• Serves as patient advocate and case manager during all phases of transplantation; documents care accurately and timely.
