Job summary

Wellstar Health System is seeking a transplant coordinator RN to lead the care coordination for transplant candidates, recipients, and living donors. You will guide patients and families through education, planning and the continuum of care, collaborating with a multidisciplinary team to ensure optimal outcomes and timely documentation.

The role requires a nursing license (GA or multi-state), BLS certification, and a minimum of three years of nursing experience.

Qualifications

• Minimum 3 years nursing experience.
• GA RN license or multi-state compact.
• BLS certification (Instructor or Basic) or ARC-BLS.

Responsibilities

• Coordinate care for transplant candidates/recipients and living donors.
• Educate patients and families about the transplantation process.
• Act as case manager across pre-operative to post-discharge phases.
• Document care accurately in EMR/OTTR and ensure HIPAA compliance.
• Participate in quality monitoring and process improvement activities.
• Collaborate with multidisciplinary transplant team.
