Job summary

Saint Joseph’s Hospital and Medical Center (SJHMC) in Phoenix, AZ is seeking an RN Transplant Coordinator to lead the patient journey from referral to recovery, coordinating evaluations, education, and communication with payers and care teams.

This role requires AZ RN licensure, a BSN or higher preferred, and at least 3 years in related or critical care settings, with program management experience advantageous. SJHMC emphasizes Humankindness and clinical excellence.

Qualifications

• Graduate of accredited school of nursing.
• Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical/surgical setting.
• Registered Nurse: AZ, upon hire.
• Bachelor of Science in Nursing or higher degree preferred.
• Critical care experience preferred.
• Program management/development experience preferred.

Responsibilities

• Organizes the clinical program.
• Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population.
• Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.
• Serves as resource to hospital staff regarding patient care.
• Coordinates the patient evaluation process.
• Performs initial assessment of patient and family according to protocol for acceptance into program.
